Philip (Phil) Krukow is a dedicated Physician Assistant graduate from Rocky Mountain University of Health Professions. He completed his Bachelor’s Degree at the University of Utah while gaining hands-on experience working as a Medical Assistant in multiple specialties with the U of U’s Department of Pediatrics at Primary Children’s Outpatient Clinic in Salt Lake City.
Mimi Alhafid is a dual-certified Family Nurse Practitioner and Psychiatric Mental Health Nurse Practitioner who earned her master’s degrees from Rocky Mountain University of Health Sciences. Prior to becoming a Nurse Practitioner, Mimi spent six years working as an ICU nurse, where she gained extensive experience caring for patients with complex medical needs.
Ryan Farr is a Clinical Mental Health Counselor with over 15 years of experience helping individuals work through anxiety, depression, substance use, and major life transitions. Using evidence-based approaches like Cognitive Behavioral Therapy (CBT), Ryan is known for his grounded, supportive style and his focus on helping clients build stability, resilience, and lasting personal growth.
Hailey Edmonson is a Substance Use Disorder Counselor (SUDC) who provides compassionate, trauma-informed care to individuals navigating recovery and mental health challenges. She specializes in supporting clients through substance use treatment, medication-assisted treatment (MAT), and co-occurring disorders while helping them build healthy coping skills and long-term stability in a safe, supportive environment.
Yes we currently offer walk-in hours of 8am to 4pm Monday to Friday. Outside of those hours, please call (435) 248-0333 ahead of time for availability, to ensure less waiting. Otherwise we’ll fit you in where we can. Check our office hours.
